The Health Information Services (H.I.S.) department staff is dedicated to the effective management of personal health information needed to deliver quality healthcare to you. Quality information is essential to all aspects of today's healthcare system. Health information management (HIM) is the body of knowledge and practice that ensures the availability of health information to facilitate real-time healthcare delivery and critical health-related decision making for multiple purposes across diverse organizations, settings, and disciplines.
Medical records are an important part of your health care. These records are a written history of your health condition and treatment. They are used by doctors, hospitals, health care clinics, and other health care practitioners and facilities to treat you. The Health Information Services Department manages and organizes all the reports and documents that make up your health information at NRMC. The department works with physicians and many other departments in the facility to provide information that is utilized for quality care and patient outcomes.

Privacy and Release of Information
The Health Information Services department is responsible for ensuring that patient privacy and compliance with HIPAA federal regulations are met. We take every precaution to ensure your protected health information is secure. You may access and request health information by completing an “AUTHORIZATION TO USE AND/OR DISCLOSE HEALTH INFORMATION.” Click here to download and print a copy of the form. Forms may be faxed or delivered to Health Information Services. There is a fee for copying of records and identification is required when picking up records.
Birth Certificates
The Health Information Services and OB nursing staff assist you in completing appropriate paperwork to receive birth certificates. Information is gathered and forwarded to the State Bureau of Vital Statistics where your original birth certificate is made. Copies or replacement Missouri birth certificates may be obtained from the Vernon County Health Department at 301 N. Washington for a $15.00 fee.
Coding and Reimbursement
The Health Information Services staff is responsible for accurately coding your health record based on physician documentation. If you or your insurance company has questions regarding codes you may contact our office and we will assist you in understanding and resolving issues that arise.
